Student Immunization Records

  • If your child received their vaccinations through a health care provider, please bring a copy of their yellow immunization card or their immunization record provided by the health care provider to all immunization appointments.
  • After receiving the immunization(s), remember to report and update your child’s immunization record online to ensure that we have the most up-to-date information. For more information on how to update your child’s immunization records online and what you’ll need to get started, watch this short video where a WECHU Public Health Nurse explains how parents can report their child's immunization record online.

For any questions or concerns, contact us at 519-258-2146 ext. 1222 or e-mail us at csh@wechu.org.
